Comprehensive Support in a Community Setting
We provide comprehensive customized Living services for adults facing physical and/or mental disabilities within a warm and welcoming residential atmosphere. Our facilities, conveniently situated in Minneapolis, Richfield, and Bloomington, are designed to prioritize your independence while encouraging active community involvement. Our committed team of professionals is dedicated to assisting with a variety of daily activities, including bathing, dressing, personal hygiene, grooming, and effective medication management.
We focus on community-oriented Customized Living housing and services specifically designed for adults with Serious and Persistent Mental Illness (SPMI) or other disabilities. Our locations in Minneapolis, Bloomington, and Richfield offer an array of specialized services:
Eligibility Criteria For Our Homes
To access our services, individuals must meet the following criteria:
- Age: 18 years or older.
- Diagnosis: Documented diagnosis of a mental illness, brain injury, or other disability.
- Care Plan Adherence: Willingness to follow care plans created by medical providers.
- Funding Options: We offer various funding options, including CADI, BI, and EW waivers, GRH program funding, and private funding through benefits like SSI, SSDI, or RSDI.
